Model Release Terms

I hereby grant Balance Gym, its legal representatives and assigns (including but not limited to any agency, client, or publication), irrevocable permission to publish photographs and/or videos of me taken at: Balance Gym on any date after January 1, 2023. These images/videos may be published in any manner, including (but not limited to) calendars, advertisements, periodicals, internet-connected applications (mobile apps), web applications (websites) and greeting cards. Furthermore, I will hold harmless the aforementioned photographer and his/her legal representatives and assigns, from any liability by virtue of minor cropping that may be required, and color and exposure shifts that may occur in reproducing these images or videos. I affirm that I am 18 years of age or older, and competent to agree to these terms.

Opt Out
If at any time my ability to publicly display photos of myself become restricted due to employment or other legal requirements I may request an officer of Balance Gym to have my images/video removed from any media except previously printed materials or media that Balance Gym no longer has control over.
